At Health-Span Clinics, we are committed to protecting the privacy of your protected health information (PHI), which includes any information that identifies you and relates to your health or treatment. 2. MOBILE COMMUNICATIONS AND TEXT MESSAGING

If you provide your mobile phone number and consent to receive communications via text message (SMS), we may send you informational messages such as appointment reminders, treatment updates, or account notifications. No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties. Message frequency varies. 3. YOUR RIGHTS REGARDING YOUR PHI

You have the following rights:

  • Paper Copy: Request a paper copy of this Notice.

  • Breach Notification: Be notified if your unsecured PHI is breached.

  • Authorization Revocation: Revoke any prior authorization in writing.

  • Complaints: File a complaint with us or the U.S. Department of Health and Human Services (HHS) if you believe your rights were violated (no retaliation).
